Be prepared to provide the following information:
- Project Title and Brief Description
- Principal Investigators (2), Name and Contact information
- Applying organization information, TAX ID/EIN, IRS Designation and Grant Office contact name and information

Submit your LOI by email

In two pages or less, please answer the following questions:
- What problem do you intend to solve?
- Why does the solution matter?
- How are you innovating relative to current methods and practice?
Include the following using a bullet-list format:
- Project plan
- Computational tools and data
- Assessment of performance
- Deliverables
- Implementation
- Impact
- Who benefits, by how much, and how are you measuring the benefits?
- Expected changes in clinical care practice
- Dissemination
- Proactive dissemination to other institutions and researchers
- Transfer to clinical practice
- Team
- Your team
- Computational and clinical expertise
- Preliminary work
Format:
- Up to 2 pages, plus one optional conceptual figure on a separate page
- References and biosketches separate
- Please use 11-12 pt font, 1” margins, section titles, and breaks between sections.
